12221: No air mass meter signal.
12224: Air/Fuel mix too rich
12111: Incorrect Air/Fuel mix while driving: check vacuum leaks including charcoal cannister

How to proceed?
Mass Air Meter - I can plug/ unplug repeatedly to clean connection.
Then what?
Air/Fuel too rich - might self correct with a functional Air Mass Meter??
Incorrect Air/fuel mix check vacuum leaks --> will explore charcoal cannister

Update:
all I did was to plug/unplug MAF sensor about 10-15 times.
Now:
Engine runs smooth at idle and revs to 3500 w/o issues.
No CEL anymore.
High revs from 3500 can drop to 850 RPM's at idle w/o any sputtering.

I did not do much.

Perhaps this was just a temporary fix.
Still welcoming comments.

Note:
I still cannot find the Charcoal canister even after I was told where to look by Bentley's and this forum. That is another topic posted elsewhere. I feel so stupid about that.

Have experience on this one......
Delicate exercise....
Clean the AMM pins and clean and tighten the JPT receptacles in the AMM plug.
Measure and confirm absolute "0.00" volts between AMM ground pin and engine head ground point.
Any weakness in the connectivity of the AMM plug/pins will show up with mixture error codes.
